THE DIFFERENT COUNTIES WE SERVE
GETTING IN HOME CARE IS AS EASY AS
-
Initial Call
and IntakeThe first step in our care process is conducting an initial assessment to gather information.
Then, we schedule a home meeting with the client and family. During this visit, we further assess the client's needs and develop a personalized care plan based on their medical history.
-
Personalize Care Plan
Once we've crafted a personalized care plan for our patient, we proceed to submit the required documentation to Medicaid. Following this submission, we await Medicaid's response, which typically occurs within a timeframe of 30 days, upon which approval is granted.
-
Caregiver
MatchOnce nursing services have been approved by Medicaid, we start matching our caregivers and nurses with the patient. Caregivers and nurses will keep a notebook for all care notes. Also, a supervisory nurse makes regular visits during care hours to ensure quality.
