At Gentle Care Home Services, we work with families of medically fragile individuals up to age 21, providing access to 24/7 home care services, including feedings, catheter and ventilator care, medication management, and mobility assistance. All of our services are provided at no out-of-pocket costs for eligible families, allowing you to spend more time making memories and less time managing your child's care. How Do You Provide Care for Medically Fragile Youth?
We'll begin with a thorough review of your child's condition, medical history, and care needs, all of which will be used to construct a personalized home care plan to enhance their comfort and empower them to engage with the world safely and confidently. All required documentation will be sent to Medicaid for review. Upon approval (which usually happens within 30 days), we'll match your child with nurses or caregivers and start our services. We'll keep comprehensive notes on your child's care and progress with a supervisory nurse making regular visits to ensure they're receiving the quality care they deserve. We'll also adjust your child's care plan as they grow. With 24/7 support available, our team is here to help your family navigate the challenges that come with caring for a medically fragile child.
